What Caused This Fatal Collision?

There are a number of unanswered questions here. What caused the collision? Who was at fault?

Did the driver of the vehicle fail to yield the right-of-way to the bicyclist? Did the motorist intrude into the bike lane? Did this accident occur in a crosswalk? Did alcohol, drugs, speeding or distracted driving play a part in this collision?
